$80 for recreational; $125 for classic
Spring-only fees: $45 recreational; $70 classic
Alliance Development Program (ADP) (pdf)
Registration form for the Alliance Development Program (ADP) for youth in grades 1-4, run by the Madison 56ers. Cost is $95 for the fall and spring seasons. Please direct all questions to Ryan Uphoff at the Madison 56ers (56ersAsstDir@gmail.com).
Recreational Coaches' Forms:
Team Players Fee Form (pdf)
Use this form to list all the players returning to your team with the fees paid. Please indicate if the player's fee is waived because of a coach exemption or scholarship.
Team Jersey Order Form (pdf)
For teams that have new players who require jerseys -- for use only by the coach or manager. Please see the form for more details.
Regent In-House Team Registration Form (pdf)
For U7-U10 teams. Teams are scheduled by Regent and play other Regent, Shorewood, Magic and/or Verona teams.
MAYSA Recreational Team Registration Form (pdf)
For U11 and older recreational teams. Teams are scheduled by MAYSA and play other teams in the greater Madison area.

Manager's Uniform Spreadsheet (excel)
Used to compile uniform orders from individual players. Manager uses to submit orders to Stefan's Soccer.

Individual Classic Uniform Flyer (pdf)
Flyer shows the style of the 2007/08 Classic uniforms. All teams must purchase new uniforms as the style has changed from last year. The form is used to order the uniform and must be turned into the team manager.
